How do you make an appointment?
Preferably use the form below, but you can also make an appointment by telephone or by e-mail for a consultation or treatment.

Telephone times are from 8.00-9.00 am and 3 – 4 pm daily (Monday to Friday)
Outside these times the telephone may not be answered because we are busy (not to disturb the treatments). We kindly ask you to speak into the answering machine. If you give us your name and your telephone number, we will call you back.

You can indicate your preference for a certain physiotherapist at registration. This is taken into account.
For registration we will ask you for some basic information. We will then call you back.
What do you bring with you?

On your first visit you bring a referral letter with you from your general practitioner, specialist or company doctor if you have received one.
If you do not have a referral, we will apply a screening on your first visit to see if your complaint is eligible for treatment. This is part of the Direct Accessibility Physiotherapy.
We will also ask you for your insurance card and proof of identity.

If you cancel at least 24 hours in advance if you are unable to attend, the reserved time will not be charged.
